This section includes the fields in the API that fullfill the requirements of the Disclosure Questions section of the CAQH form.

Affirmative Answers With Explanations
Providers have the option to include an explanation and/or document for any affirmatively answered disclosures. Affirmatively answered disclosures will have two additional keys containing supplemental information given by the provider. The keys will match the key for the boolean value with explanation or document appended.

For example, if the value of attested.caqh|licensure.revoked is True, there will be two additional keys in the disclosures section, the explanation key will be attested.caqh|licensure.revoked.explanation and will contain the optional explanation.

attested.caqh|licensure.revoked(boolean) -- Practitioners attestation with regards to license revocation.

attested.caqh|licensure.challenged(boolean) -- Practitioners attestation with regards to challenges of their license(s).

verified.npdb.state_licensure_action(boolean) -- Presence of state licensure actions (inclusive of revocations and challenges) on the practitioners' NPDB report.

You can also check individual state licenses for the presence of actions.

attested.caqh|privileges.revoked(boolean) -- Practitioners attestation with regards to privilege revocation.

attested.caqh|privileges.surrendered(boolean) -- Practitioners attestation with regards to surrendering of privileges.

attested.caqh|privileges.terminated(boolean) -- Practitioners attestation with regards to termination of privileges.

verified.npdb.clinical_privileges_action(boolean) -- Presence of privilege actions (inclusive of revocations and challenges) on the practitioners' NPDB report.

You can also check individual hospital affiliations for the presence of privilege revocation or absence of good standing.

verified.npdb.peer_review_org_actions(boolean) -- Presence of peer review actions on the practitioners' NPDB report.
