This section includes the fields in the API that fullfill the requirements of the General section of the CAQH form.

attested.basic_info.gender(string) -- The gender of the practitioner as indicated by the practitioner for the purposes of the NPDB.

verified.axuall_id.date_of_birth(date) -- The date of birth of the practitioner. This field is verified by an approved method of ID verification.

attested.basic_info.ssn(string) -- The social security number of the practitioner.

verified.basic_info.phone_number(string) -- The phone number of the practitioner.
