This section includes the fields in the API that fullfill the requirements of the Professional IDs section of the CAQH form.

DEA License

verified.dea.number(string) -- The DEA number of the practitioner. This number has been verified using the DEA as a primary source.

verified.dea.expiration_date(date) -- The expiration date of the DEA license of the practitioner. This number has been verified using the DEA as a primary source.

verified.dea.state(string) -- The DEA state of registration of the practitioner. This number has been verified using the DEA as a primary source.

verified.dea.payment_indicator (string) -- The DEA payment indicator of registration of the practitioner. This number has been verified using the DEA as a primary source.

verified.dea.drug_schedule (string) -- The DEA drug schedule of the practitioner. This number has been verified using the DEA as a primary source.

verified.dea.activity(string) -- The DEA activity of the practitioner. This number has been verified using the DEA as a primary source.

verified.dea.state(string) -- The DEA state of registration of the practitioner. This number has been verified using the DEA as a primary source.

verified.dea.patients_permitted (string) -- The DEA patients permitted (DEA-X only) of the practitioner. This number has been verified using the DEA as a primary source.

verified.dea.uin (string) -- The DEA UIN (DEA-X only) of registration of the practitioner. This number has been verified using the DEA as a primary source.

verified.dea.address(dict) -- The DEA address affiliated with the registration of the practitioner. This number has been verified using the DEA as a primary source.

verified.dea.artifact(url) -- Axuall DEA Proof artifact. This is usually an image of the practitioners drivers license. This link will expire within 10 minutes of when the API call was made.

verified.dea.zkp(json) -- Axuall Digital Proof - This proof can be used to ensure the authenticity, and current status of the verified data against the credential definition on the Sovrin network.

verified.dea.meta(json)-- Verification metadata - This object contains information about how and when the data was verified.

State License(s)

The Axuall API will return an unbounded array of verified and active state licenses in this section. The subsequent section documents the fields within a single state license element.

verified.state_license.number(string) -- The State medical license number of the practitioner. This number has been verified using the pertinent State license website.

verified.state_license.state(string) -- The issuing state of the State medical license of the practitioner. This information has been verified using the pertinent State license website.

verified.state_license.expiration_date(date) -- The expiration date of the State medical license of the practitioner. This information has been verified using the pertinent State license website.

verified.state_license.license_status(string) -- The status of the State medical license of the practitioner. This status has been verified using the pertinent State license website.

verified.state_license.board_actions(string) -- The presence of board actions of the State medical license of the practitioner. This status has been verified using the pertinent State license website.

attested.basic_info.occupation_code(string) -- The type of practitioner as attested to by the practitioner.

verified.state_license.artifact(url) -- Axuall State License artifact. Link to the most recent downloadable screenshot of the State medical license website. If there are state actions, they will be appended to the screenshot. This link will expire within 10 minutes of when the API call was made.

verified.cds.issue_date(date)-- The issue date of the CDS license of the practitioner as verified against the pertinent CDS website.

verified.cds.state(string)-- The issuing state of the CDS license of the practitioner as verified against the pertinent CDS website.
